The Transitional Care Unit at Hackensack Meridian Health Mountainside Medical Center is a licensed Skilled Nursing Facility that offers all of the services and amenities found at stand alone nursing facilities along with unique benefits derived from its hospital campus location. Admission is open to all patients who meet our short-stay criteria.
Our Transitional Care team provides compassionate, expert post-acute care that’s an integral step in the road to recovery for some patients. Our Transitional Care Unit (TCU) is one of a very small, select group of facilities statewide that specializes in short-term rehabilitation limited to eight days.
Staffed by a team of skilled, board-certified physicians and other health professionals, the TCU serves as a bridge between hospital and home for patients recovering from an injury, illness or surgery.
Admission is open to qualified patients from all hospitals and our specialists will communicate with your personal physician about your progress and coordinate your care throughout your stay and upon discharge.
